• Photo size must be 1.5” x 1.5”
• Photo must be recent, color, head and should be passport-style photo no older than 6 months. ( scanned and computerized pictures not allowed)
• Student must be facing the camera with a plain white background. No side profiles!
• The background must not have any other objects showing or the photo will be rejected.
• No sunglasses, hats, funny faces, etc. Headpieces only if worn for religious purposes.
• There can be no borders on the photo.
• The picture must clearly identify you and be of good quality.
• The student has to sign in full in the box provided. Since the signature is proof of identity, it must be genuine and in full; initials are not sufficient. Signature in CAPITAL LETTERS is not permitted.
• Ensure that the signature is within the box otherwise it will not be processed.
• The signature must be signed only by the student and not by any other person.
